   The covid-19 vaccines are getting more available to everyone. As of right now, clinics are opening the vaccine to walk-in appointments, and people of ages 16 and older are able to receive the vaccine. In addition, specific work groups are able to receive the vaccine. The specific health guidelines and vaccine directions, directly taken from the new york state website, are as follows:

Eligible New Yorkers are:

  • Age 16 and older
  • Reside in New York State
  • Work in New York State
  • Study in New York State

 

Vaccination Instructions

Below are instructions for receiving your vaccine at New York State-Operated Vaccination Sites. New York State mass vaccination sites are now also accepting walk-in appointments for all eligible adults.

Step 1: Determine eligibility and schedule an appointment. The Am I Eligible app is the quickest way to see if you’re eligible and make an appointment. You can also call the New York State COVID-19 Vaccination Hotline at 1-833-NYS-4-VAX (1-833-697-4829). Once you have successfully scheduled an appointment, you will receive a confirmation email that contains a barcode. You will need to bring this to your appointment.

Step 2: Complete the Vaccine Form. Once you have a confirmed appointment, you must complete the New York State COVID-19 Vaccine Form. This form should be filled out online and you will receive a submission ID indicating completion. You will need to bring the submission ID to your appointment. If you cannot submit the form online, it will be available at the vaccination sites.

Step 3: Bring proof of eligibility to your appointment. New York State residents must bring proof of identity. For minors under 18, a parent or guardian is required to identify the minor. If you are not a New York resident, you must bring proof that you work or study in New York State. You will be asked for insurance information BUT the vaccine is free and there will never be a charge to you. This information is for administrative use only.

Step 4: If you received the Pfizer or Moderna vaccine, your second dose appointment will be scheduled automatically when you receive your first vaccine dose. Your second appointment will be scheduled for the same time and at the same location, three weeks following your first dose. You will receive a card onsite with the date and time indicated and a confirmation email will follow a few days later. Please keep in mind when scheduling your first appointment that your second appointment will occur at the same time of day. The Janssen/Johnson & Johnson vaccine only requires one dose.

 

Acceptable Forms of Proof of Eligibility

Identification:

Sufficient identification includes a driver’s license, passport, or any legal proof of your date of birth and residency. Note that non-NYS residents may be vaccinated if they work in NYS. For individuals under the age of 18, a parent or legal guardian must provide consent for vaccination. A parent or legal guardian must provide verbal consent either in person, or by phone, at the time of vaccine appointment.

Documentation of age is required, and can be a Driver’s license or non-driver ID; Birth certificate issued by a state or local government; Consulate ID; Current U.S passport or valid foreign passport; Permanent resident card; Certificate of Naturalization or Citizenship; Life insurance policy with birthdate; or Marriage certificate with birthdate.

Proof of NYS Employment:

  • an employee ID card or badge,
  • a letter from an employer or affiliated organization, or
  • a pay stub.

Guiding Principles

New York State based its COVID-19 vaccine distribution and administration process on ten guiding principles.

  1. Safety: New York State will only endorse and distribute a COVID-19 vaccine if it is determined to be safe and will only be used according to the indication under which it received its authorization or license. This includes continued monitoring and reporting of adverse events after the vaccine is licensed and administered.
  2. Effectiveness: New York State will only endorse and distribute a COVID-19 vaccine if it is demonstrated to be appropriately effective in the populations intended for use.
  3. Expert approved: New York State will rely on the advice and counsel of recognized clinical experts and scientists to review and approve the safety and effectiveness of every vaccine that is authorized by the federal government for distribution.
  4. Equitable & clinically driven distribution: New York State’s COVID-19 vaccine distribution plan will be based on standards that prioritize people at higher risk of exposure, illness and/or poor outcome. Unrelated factors, such as wealth or status, will not influence distribution.
  5. Transparency: Throughout the COVID-19 crisis, the state’s daily public presentation of facts and reliance on science and medical expertise helped build public trust and confidence in government action. New York State will continue to be transparent regarding all aspects of the COVID-19 vaccine distribution, administration, and monitoring process to ensure New Yorkers are fully informed.
  6. Use of Data: Coordination of a successful vaccination program will require robust tracking, data and analytics capabilities. New York State will use powerful data and information technology platforms to guide all parts of the COVID-19 vaccine distribution and administration process to maximize safety, accuracy, and efficiency and meet all federal reporting requirements — all while maintaining patient privacy.
  7. Privacy and Patient Safety: New York State will ensure all vaccination processes prioritize patient safety, and all information systems guarantee patient privacy. Vaccination does not take away the importance of other public health measures that have served us well in the fight against COVID-19. New Yorkers will be urged to continue to practice social distancing, mask wearing, hand washing, and other measures.
  8. Partnership, Coordination & Public Outreach: New York State recognizes that coordination with local organizations and community providers is essential to the safe and successful distribution and administration of COVID-19 vaccines. The state’s outreach efforts will especially focus on reaching underserved, hard to reach, and vaccine-hesitant populations, as well as those at highest risk for COVID-19 infection and poor outcomes.
  9. State Leadership: New York State expects federal engagement on vaccine vetting, distribution, administration, and funding. However, regardless of the level of federal engagement, New York State will take all necessary steps and require local coordination with the state’s centralized approach to ensure an efficient and organized vaccine distribution.
  10. NEW YORK TOUGH: Throughout this COVID-19 crisis, New Yorkers have shown that there is nothing we cannot do if we work together as one community. Our approach to the COVID-19 vaccine will be tough, strong, united, disciplined, and loving.

Updates

The Centers for Disease Control and Prevention (CDC) and the U.S. Food and Drug Administration (FDA) officials on April 13, 2021, recommended pausing administration of the Johnson & Johnson COVID-19 vaccine “out of an abundance of caution,” and mainly for the benefit of healthcare providers to diagnose, treat, and report severe adverse events.

In compliance, Suffolk County Department of Health Services has paused dispensing of the Johnson and Johnson COVID-19 vaccine at its vaccination clinics, while the CDC and FDA review data about an extremely rare type of blood clot that has been reported in six people who received that vaccine in the United States.

Suffolk County officials are not aware of any cases occurring among the approximately 2,000 residents who have received this vaccine through the County Health Department thus far. Those residents who received the Johnson & Johnson vaccine who develop severe headache, abdominal pain, leg pain or shortness of breath within three weeks after vaccination should contact their health care practitioners promptly.
